Yearly major bleeding risk on anticoagulation, and what to fix.

TellBleeding on a blood thinner comes from fragile vessels, a weak plug, or too much drug, and most of the points are things you can change.

Uncontrolled blood pressure (systolic over 160)+1
Abnormal kidney function: dialysis, transplant, or creatinine 2.26 mg/dL or higher+1
Abnormal liver function: cirrhosis, or bilirubin over 2 times normal with AST, ALT or alkaline phosphatase over 3 times normal+1
Prior stroke+1
Prior major bleeding, or a bleeding tendency such as anemia or low platelets+1
Labile INR: time in range under 60% (warfarin only)+1
Age over 65+1
Taking an antiplatelet or an NSAID+1
Alcohol: 8 or more drinks a week+1
